CALL OUT AND DIAGNOSIS CONTRACT
Parties:
This contract (“Contract”) is entered into between Mr Hot Tubs Limited, hereinafter referred to as “Service Provider,” and yourself, hereinafter referred to as “Client.”
Scope of Services:
The Service Provider agrees to provide call out and diagnosis services to the Client for issues related to hot tubs and related equipment.
Fees:
- Call Out and Diagnosis Fee:
- The Call Out and Diagnosis Fee is £85 plus VAT for hot tubs sold and installed by Mr Hot Tubs. For hot tubs supplied by third parties the call out is £120+vat. This is within a 20 mile radius of Service Provider’s Base.
- Distance Surcharge:
- For locations beyond 20 miles from the Service Provider’s base, a distance surcharge of £80 plus VAT per hour will be applied, this is until return to base.
Payment Terms:
- The Call Out and Diagnosis Fee is payable by the Client upon the Service Provider’s invoice.
- The Distance Surcharge, if applicable, will be calculated based on the actual distance from the Service Provider’s base to the Client’s location and will be invoiced separately. Payment for the Distance Surcharge is due on the day of invoice.
Cancellation Policy:
- If the Client needs to cancel a scheduled call out and diagnosis appointment, a minimum notice of 24 hours is required. Failure to provide timely notice may result in a cancellation fee equivalent to 50% of the Call Out and Diagnosis Fee.
Terms and Conditions:
- The Service Provider will perform a diagnostic evaluation of the hot tub and provide a report to the Client.
- Any repairs or additional services required beyond the initial diagnosis will be subject to a separate agreement and additional fees.
- The Call Out and Diagnosis Fee covers the Service Provider’s travel time, initial assessment, and diagnosis only.
- The Client agrees to provide the Service Provider with reasonable accessible access to the hot tub and related equipment. The access should not include additional decking or framework.
- The client agrees there is sufficient space for accessing and working on the hot tub. If access the hot tub is not accessible The Client is still responsible for paying the Call Out and Diagnosis Charge.
